Organizations with programs or proposals consistent with the priorities of The Baxter International Foundation may apply for a foundation grant. Strong preference is given to projects that are brought to the foundation through a local Baxter facility or employee. If you have questions about whether your project falls within our guidelines, please contact the foundation via e-mail at fdninfo@baxter.com before submitting a proposal. An application form is not required, but all proposals must include the following information:
- The organization's full legal name, complete mailing address, phone number, fax number, contact person's name and e-mail address.
- If applicable, a description of the organization's relationship with the local Baxter facility or employee.
- A brief description of the organization, its purpose, history, programs and achievements.
- A statement describing the specific purpose of the grant requested, including how the project meets the priorities of The Baxter International Foundation.
- A statement of the amount of money requested in U.S. dollars and over what period of time.
- A plan for how the program or services will be sustained after the conclusion of funding from The Baxter International Foundation.
- A plan for measuring results and reporting periodic progress, as well as for a final evaluation.
- Financial information, including the organization's current operating budget and the budget for the proposed project.
- A list of sources of income and amounts, including support from corporate donors, foundations, the United Way and the government – committed and approaching.
- Audited financial statements for the most recent fiscal year and the two previous years (total of three).
- A copy of the organization's certificate of tax exemption for the Internal Revenue Service as a 501c3 or equivalent organization. For organizations outside the United States, a copy of the charitable certificate and exemption from taxes from the local or national government. Upon initial review of the request, organizations outside the United States will be sent an additional inquiry form.
- A list of officers and board members, with their home addresses and their affiliations. See “Compliance” information below. *
- A list of the names and home addresses of the five highest paid employees. *

In general, The Baxter International Foundation does not make grants to:
- Capital and endowment campaigns (includes requests for infrastructure of any kind, equipment, vehicles, etc.)
- Disease-specific organizations.
- Educational institutions, except in instances where a grant would help achieve other goals, such as increasing community-based direct health services or the skills and availability of community health-care providers.
- Hospitals.
- Individuals, including scholarships for individuals.
- Organizations with a limited constituency, such as fraternal, veterans or religious organizations.
- Lobbying and political organizations.
- Organizations soliciting contributions for advertising space, tickets to dinners, benefits, social and fund-raising events, sponsorships and promotional materials.
- Organizations seeking travel support for individuals or groups, medical missions or conferences.
- Magazines, professional journals, documentary, film, video or radio productions.

| Application Review Process |

Proposals are reviewed on a quarterly basis. Foundation grants will address critical needs nationally, focusing on communities where Baxter has a corporate presence. In Illinois, grants are restricted to Lake, McHenry and Cook counties. The foundation also funds in Asia, Australia, Canada, Europe, Latin America and Mexico. In evaluating a grant application, The Baxter International Foundation looks for:
- Eligibility under foundation guidelines.
- Evidence that the project is a response to valid need and is superior to other competing projects.
- Evidence of the organization's capacity to accomplish its goals.
Preference is given to proposals that expand or improve access to healthcare with a strong plan for sustaining the services or program beyond the term of The Baxter International Foundation grant. Grants are not routinely renewed on an annual basis.
Occasionally, multiyear proposals may be considered. Multiyear commitments still require submission of an annual request, with a financial statement and narrative account of what was accomplished with the grant funds, a description of progress made toward achieving the goals of the grant and a work plan for the coming year. |

Qualified proposals must be received in the Foundation office before or by the end of the business day of the deadline dates below:
- 1st Quarter - deadline of January 21, 2008
- 2nd Quarter - deadline of April 1, 2008
- 3rd Quarter - deadline of July 1, 2008
- 4th Quarter - deadline of September 26, 2008
Final approval for foundation programs is made by the foundation's board of directors, which meets quarterly.

| The Baxter International Foundation Voluntary Guideline Principles |

To assist in the efforts of the United States government to prevent the financing of terrorism, many U.S. charities, foundations and corporations have undertaken additional procedures in their due diligence review of prospective grantees. As is the case with most U.S. foundations, the review process for grant requests for The Baxter International Foundation includes checking the names of our grantees against lists of terrorist organizations maintained by the U.S. government and other organizations such as the European Union and the United Nations. This process will include checking the names of the principal officers, directors and staff of the organizations to whom we are considering awarding grants.
In this process we are guided by the following principles:
- We respect the government's right to take appropriate steps to prevent the financing of terrorism and acknowledge that the charitable sector must do its part to ensure that charitable funds do not inadvertently fund terrorists or their organizations.
- The Baxter International Foundation has always undertaken due diligence to ensure grants are used for their intended charitable purposes. We believe our grantees perform vital and important work all over the globe to improve health and human needs that is unrelated to actions, methods or purposes advocated by terrorist organizations.
- The facts and circumstances of particular grant requests may require that we undertake additional due diligence or require that our grantees provide additional information and assurances regarding the use of our grant funds.
- We strive to apply these measures consistently.
We believe these principles will result in the right balance between the legitimate needs of the U.S. government to prevent the financing of terrorist activity and the desire to support the ability of our grantees to perform important work. We are committed to working with our grantees to address any issues that may arise, while doing our best to ensure our practices are in accord with applicable laws and regulations. In the unlikely event that questions arise from this process, we will seek to ensure that the information is accurate. The more information provided allows for the reduced chance of “false positive” information appearing. This is one reason why we ask for information such as home addresses. All information provided to us is kept confidential and used exclusively for the purposes described in these principles. This information is not released to any other organization, or used by Baxter or any affiliates to contact any of the individuals.
